This form is the National Form and does not include Chapter
affiliation information. Please indicate Chapter affiliation
if transferring into the TNSSAR. The TNSSAR has no at-large
membership and all members must be affiliated with a TNSSAR
Chapter. The Chapter dues are included in the TNSSAR dues payment.
This is a PDF Acrobat Reader form that may be filled in on computer
and printed for mailing.

in and print it, Transfer
Form.
For members transferring into the TNSSAR we ask that the
mailing of the form be done in reverse order to allow for followup
and tracking. Please prepare the form and then forward the form,
with either a $23.00 check, payable to the TNSSAR, along with
a record copy of your original application or a $28.00 check,
payable to the TNSSAR, in order that a record copy ($5.00 fee)
may be ordered from HQ NSSAR, to the current TNSSAR VP Data
Management (see Contacts for address).
A transmittal (and $5.00 check if required) will be prepared
and forwarded, with the TN completed form919, to HQ NSSAR via
the Secretary of the current State Society.
